This is a monthly perpetuity membership option for those interested in regular, ongoing care who have completed the intensive 4 month initial membership.  Typically we will meet monthly for 45 minutes.  This subscription includes labs, supplements and medications billed at cost (wholesale pricing- savings passed on directly to the patient).  Additional care time is billed at the regular consult rate of $500/hr.   We do require a minimum of 4 months in the initial membership model before moving to  this monthly option to best optimize your health.  The cost is $350/mo.  Additionally, you have the option to move to my primary care practice, which is now closed, for $109/mo or the  functional medicine primary care combined practice for $459/mo.  This is ideal for those who want to include their primary care needs at the same practice.   For both options, all discounted labs, medications and supplements will continue.  

 

If you have your own primary care physician and only want to continue with functional medicine as a supplement, that rate is $350/mo and does not include any primary care and you must have a primary care physician monitoring your other healthcare needs.
